About us

Our Model

The Twin Cities Community Health Worker Hub is a community-led collaborative being incubated by Pillsbury United Communities—connecting the dots between community health organizations, wraparound social services, and the various public and private funding streams that make their work possible.

The Hub offers a single point of access for healthcare partners to refer individuals for care. Building on the well established Pathways Community HUB model and utilizing an outcomes-based payment structure, the Hub aims to reduce disparities in health outcomes for communities of color, low-income communities, and other historically underserved populations.

Partnerships

CHW PARTNERS

Our work is centered around community health workers (CHWs) and the organizations who support them. Our CHW partners enable their health workers to deliver care, supported by the Hub’s wraparound services and braided funding model.

REFERRAL PARTNERS

Referral partners work with CHWs, but do not employ them directly. When our referral partners meet community members who may need a CHW, they are able to refer those community members to the Hub for easy access to our full network of partners.

RESOURCE PARTNERS

Resource partners provide critical wrap-around services and care in community. These can include government entities or human services organizations. These are important partners to CHWs on the ground, who draw on these vital resources to ensure our neighbors’ needs are fully met.

FUNDING PARTNERS

Payers (health insurance companies) are committed to investing in the care that CHWs deliver, particularly through outcomes-based payment. Additionally, private and public funders provide braided funding support to sustain the hub. We’re also seeking innovation funding partners willing to support the Hub’s infrastructure development.

FUTURE PARTNERS

Your organization may not be ready for any of the defined partner roles. That’s okay! We welcome collaborative ideation on how you might serve as a partner in the Hub network in the future. Let’s make an introduction and see where that takes us.
